High experimental and under development.

When building a mesh on the CPU and running assembly/solve on a GPU, it is natural to use one MPI rank per GPU. Typically, for each GPU on a node there will be many more CPU cores. Therefore, it is helpful to use threading to speed up steps that are best performed on the CPU.

The change starts to introduce threading in mesh construction.
