Primarily version bumps and additional console output; the only behavioral change is adding APP_VERSION to the CLI-spawned server environment.

Overview
Bumps the repo, api, web, @betterdb/shared, and CLI package versions to 0.4.5.

Adds a GitHub “star us” prompt to both the API server startup logs (apps/api/src/main.ts) and the CLI startup output via a new printGitHubInfo() helper.

Updates the CLI runner to read its package.json version and export it to the server process as APP_VERSION.
