Need to check the following cases and ensure they match between ctrmml and mdsdrv

• 1A) Tie before the first note on FM channel
• 1B) Instrument change followed by tie before the first note on FM channel
• 1C) Note, instrument change followed by tie on FM channel
• 1D) Rest followed by tie on FM channel
• 1E) Rest followed by instrument change then tie without new note on FM channel
• 2A) Tie before the first note on PSG channel
• 2B) Instrument change followed by tie before the first note on PSG channel
• 2C) Note, Instrument change followed by tie on PSG channel
• 2D) Rest followed by tie on PSG channel
• 2E) Rest followed by instrument change then tie without new note on PSG channel
• 3A) Tie before the first note on PCM channel
• 3B) Instrument change followed by tie before the first note on PCM channel
• 3C) Note, Instrument change followed by tie on PCM channel
• 3D) Rest followed by tie on PCM channel
