This is not a standalone package and should be set up with the releases project that can be found here:

https://github.com/geonition/releases

You can try that one but I cannot promise you any easy way to set it up due to the system complexity.

May I ask in what sense are you interested in the geoforms package? Do you need map based questionnaires?
