add destroy method

There are issues with settings being carried over when rendering a new chart in some situations. This would be resolved by calling codebook.destroy() on a previous version before initiating a new codebook on the same page.

codebook.destroy() should remove all summary table charts, the data listing, and the chart maker chart.
