Enhance bookmarks to deep link to specific content

Currently, bookmarks only save references without navigation. We should:

1. Store URL params with each bookmark (?subject=...&module=...&topic=...)
2. Make bookmarks clickable to navigate to the saved position
3. Highlight the bookmarked content when reached
